Express Scripts strikes partnership with Prime Therapeutics



Pharmacy service providers Express Scripts and Prime Therapeutics launched a three-year partnership to negotiate lower drug costs. The deal would allow Prime to use Express Scripts’ retail pharmacy network and pharmaceutical contracts.

Combined, the two companies will be able to leverage a large customer base. Express Scripts, the large pharmacy benefit company that Cigna acquired last year, covers more than 75 million customers across 3,000 health plans. Prime Therapeutics, which is owned by 18 Blue Cross and Blue Shield plans, covers 28 million customers across 23 health plans.

Express Scripts President Tim Wentworth said in a news release that the two companies could use their capabilities to deliver more affordable care.

Prime CEO Ken Paulus added the collaboration “…will improve outcomes while still maintaining flexibility and transparency to the clients we proudly serve.”

The two PBMs claimed the partnership would result in more affordable care for clients and plan members, though they did not specify how much it would save them. Express Scripts expects the deal will begin to contribute to its income starting in 2021. Last year, Express Scripts contributed $2.6 billion to Cigna’s pharmacy revenues.

“How members see savings is a decision that ultimately resides with payers but typically savings are used to control overall premium rates,” Express Scripts Spokeswoman Jennifer Luddy wrote in an emailed statement. “Additionally, once implemented, we do expect some members, especially those in high deductible health plans or with co-insurance, will benefit from lower costs on medicines at their pharmacies.”

Both companies will still negotiate independently with pharmaceutical managers, and the companies will also separately manage relationships related to value-based contracting. Relationships with caregivers, members and other stakeholders will also remain independent.

The news follows heavy consolidation between PBMs and health insurance plans. Cigna closed its $67 billion purchase of Express Scripts at the end of 2018, and a federal judge approved the CVS-Aetna merger in September. Most recently, UnitedHealth Group announced plans for its subsidiary OptumRx to acquire specialty pharmacy company Diplomat for $300 million.

Tandem Diabetes snags FDA approval for interoperable insulin pump software


Tandem Diabetes Care received FDA approval for a new predictive dosing software that can work with Dexcom’s continuous glucose monitors.


Faced with growing competition in the market for diabetes devices, Tandem Diabetes Care’s stock shot up in February when the U.S. Food and Drug Administration classified its insulin pump as the first in a new device category for interoperable insulin pumps. The San Diego-based company got an early present in December when its newest technology, a predictive software designed to keep users within the optimal blood sugar range, received a de novo approval from the FDA.

The technology went through the FDA’s new interoperability pathway, which encourages device-makers to build products that can be used across systems or platforms. Both Tandem Diabetes’ software and its t:slim X2 insulin pump are designed to work with Dexcom’s continuous glucose monitors, with the ability to predict blood glucose levels 30 minutes in advance and increase or decrease insulin levels accordingly.

The Dec. 13 approval designated Tandem Diabetes’ software as the first device in a new category for interoperable automated glycemic controllers, paving the way for future approvals in the category.

In the future, other companies with equivalent technologies will be able to go through the FDA’s 510(k) review process, said Dr. Tim Stenzel, director of the FDA’s Office of In Vitro Diagnostics and Radiological Health.

In a news release, Stenzel said the approval was a continuation of the FDA’s work “…to help ensure the safety and efficacy of innovative and customizable diabetes management systems that may help patients better tailor their treatments to their individual needs.”

The approvals this year marked good news for Tandem, after it recovered from a plummeting stock price two years ago as competitors took a larger slice of the market. In mid-2017, the company’s stock fell below $1 per share. Now, the company’ stock is valued at $60.53 per share.

“With this clearance, we will be launching the most advanced automated insulin dosing system commercially available in the world today,” Tandem Diabetes CEO John Sheridan said in a news release. “This is a testament to our commitment to improving the lives of people with diabetes by offering simple-to-use products that deliver superior performance.”

For the approval, Tandem Diabetes submitted the results of a randomized clinical trial that involved 168 participants with type 1 diabetes. Patients either used insulin pumps installed with Tandem’s new Control-IQ Technology controller, or they used a continuous glucose monitor and insulin pump without the controller.

According to the study, the controller was able to determine safe and effective insulin delivery with limited user intervention outside of mealtimes. The FDA also evaluated the controller’s ability to communicate with all parts of the system, as well as its reliability and cybersecurity measures.

“Not only do new closed-loop systems need to be effective at improving glycemic control, they must also be easy to understand and use so patients can experience the full benefits of the technology,” Boris Kovatchev, principal investigator for the trials and director of the University of Virginia’s Center for Diabetes Technology, said in a news release. “The t:slim X2 insulin pump with Control-IQ technology successfully achieved both objectives in the clinical studies.”

Tandem expects to make the new software available by the end of January.

I can admit that I’m not consistent with attending yoga classes. But, there is one yoga tool I’m committed to using just about every day: my stretching strap.

Whether you want to increase your flexibility, maintain your bend, relieve tension, or just take the edge off, adding 10 to 20 minutes of yoga strap stretching time into your self-care routine is a small way to feel really good.

This tool checks off all the boxes: it’s inexpensive (they can go for as low as £8 on Amazon) and requires minimum storage space, as well as the smallest amount of effort. Plus, it’s keeping my body warm and limber for the day I’m ready to step back in the yoga studio.

  • Lay down on your back — if you have lower back pain, bend your knees.
  • Place the middle of the yoga strap underneath the ball of your right foot.
  • Straighten your right leg and gently use the strap to pull your leg toward your body.
  • As you exhale, aim to reach or work up to a 90-degree angle for an ideal range of motion.
  • Hold at a place that feels slightly uncomfortable — but not painful — for 30 seconds if you are a beginner. If you have been stretching consistently, hold up to 60 seconds.
  • Make sure to breathe as you hold the position.
  • Exhale and release your right leg. Switch to the left side.
  • Repeat each side three to five times.
  • Reclined Open-Leg Stretch With Supine Twist For Hips

  • Lay down on your back.
  • Place the middle of the yoga strap underneath the ball of your right foot.
  • Straighten your right leg, and gently use the strap to pull your leg toward your body.
  • As you exhale, aim to reach or work up to a 90-degree angle for an ideal range of motion.
  • Grab the yoga strap with right hand, placing your left arm straight out for balance.
  • Keeping your left shoulder and hip down, open your right leg to the right side on your next exhale
  • Hold at a place that feels slightly uncomfortable — but not painful — for 30 seconds if you are a beginner. If you have been stretching consistently, hold up to 60 seconds.
  • Make sure to breathe as you hold the position.
  • Exhale and bring your leg back to the centre and cross it over to the left side of your body, keeping your right shoulder down.
  • Release your right leg back to centre. Switch to the left side.
  • Repeat each side three to five times.
  • Dancer Pose For Quadriceps

  • Make a loop at the end of your yoga strap.
  • Stand close to a wall for support, and place your right foot into the loop.
  • Bring your leg backward, and hold the strap overhead.
  • Gently walk your hands down the strap making the distance between your foot and hands shorter.
  • Continue to breathe and maintain core stability.
  • Once you reach a good stretching point, hold for 30 seconds.
  • Exhale and release your right leg. Switch sides, and repeat on the left.
  • Stretch each side two to three times.
  • Arm Circles For Shoulders

  • Hold the yoga strap with both hands slightly wider than shoulder-width apart — palms facing down.
  • Keeping your arms straight, bring the strap overhead and back, opening the chest — palms now facing up.
  • Bring the strap back forward, inhaling and exhaling as you slowly repeat the motion.
  • Move through this position for five breaths, holding at the back for five to 10 seconds.

    Sutter Health settles for $575M in antitrust lawsuit



    Sutter Health will pay $575 million to settle allegations that the Sacramento-based health system engaged in anticompetitive practices that led to higher costs for patients.

    One of Northern California’s largest health systems, Sutter Health employs roughly 12,000 physicians, and has 24 hospitals and 36 surgery centers in its network.

    Though the California Attorney General and Sutter Health previously announced the settlement in October, they did not disclose the terms until Dec. 20. If approved by the court, the sweeping terms of the settlement may set a tone for other large health systems across the state.

    “This first-in-the-nation comprehensive settlement should send a clear message to the markets: if you’re looking to consolidate for any reason other than efficiency that delivers better quality for a lower price, think again,” California Attorney General Xavier Becerra said in a news release.

    Per the terms, Sutter Health must pay $575 million to cover class action compensation and legal costs. The health system must also cease all-or-nothing contracting deals with payers, limit what it charges for out-of-network services, work with a court-approved compliance monitor for at least 10 years, and must allow insurers to provide plan members with information on pricing, quality and cost. The nonprofit must also meet clearly set definitions of clinical integration, meaning “…it must meet strict standards beyond regional similarities or the mere sharing of an electronic health record, and must be integrating care in a manner that takes into consideration the quality of care to the patient population,” according to the settlement.

    Flo De Benedetto, Sutter Health’s general counsel, said in a statement that the organization was “committed to keeping our care connected so patients continue to receive affordable, high-quality, personalized and coordinated care. Despite the increasing cost of care and operating in high-wage markets, we remain focused on making healthcare more affordable for our patients.

    “We were able to resolve this matter in a way that enables Sutter Health to maintain our integrated network and ability to provide patients with access to affordable, high-quality care. Together with the Attorney General, the parties selected an experienced monitor who will oversee the agreement, which specifies parameters for contracting between Sutter Health and insurance companies going forward,” Di Benedetto stated.

    In the future, Sutter Health will have to evaluate future capital investments based on the impact of the settlement, she added. According to Sutter Health’s unaudited financial statements, the nonprofit finished the quarter ending Sept. 30 with an operating loss of $613 million, down from a net profit of $49 million during the third quarter last year.

    The suit, originally brought against Sutter Health by the United Food and Commercial Workers International Union and Employers Benefit Trust in 2014, alleged that Sutter Health used its market dominance to prevent insurers from carving out narrower networks that would exclude some of its facilities. The case was later consolidated with a separate lawsuit filed by California Attorney General Xavier Becerra in 2018.

    According to the complaint, Sutter Health acquired significant market concentration after its acquisition of Summit Medical Center in 2000.  The California Attorney General’s complaint alleged that shortly after, Sutter Health began bundling together its providers, requiring payers to contract for them on a system-wide basis.

    Sutter also allegedly used other measures to prevent the formation of narrow networks that might exclude some of its providers, including high out-of-network costs and provisions that restricted health plans from putting its providers in any tier other than the most favored benefit tier. The health system also used confidentiality provisions that restricted health plans’ ability to provide comparisons about price and quality to plan members, according to the complaint.

    The settlement must be approved by the court before it goes into effect, with a hearing set for Feb. 25.

    I don’t want to admit to you how many times I’ve written and then rewritten this post. What can you say about a potato tian? It’s crispy! It’s delicious! There’s chipotle peppers in it! It looks fancy, but it’s really easy!

    Yukon Gold Potatoes
    I wanted to make a sweet potato tian for Thanksgiving, but I couldn’t get it right so I decided to shelve the idea for a while. When I finally revisited it, I was feeling pretty tired of sweet potatoes, so I used Yukon Gold potatoes instead. And then, finally (FINALLY!), it turned out how I had envisioned. I suspect that Russet potatoes would work out well too, but make sure you buy small ones because the big ones might be harder to pack into a baking dish.

    Freezing Leftover Chipotle Peppers

    New topic! I love using chipotle peppers in my cooking, but most recipes only call for a small amount and then you’re left with an entire can of them sitting in the fridge.

    (And you can’t throw away the leftovers! Or at least I can’t. My mom always told me that people who waste food go to hell and if I’m going to go to hell, it’s going to be for a better reason than throwing away chipotle peppers.)

    How to Freeze Chipotle Peppers
    Anyway! Here’s what you can do with your leftover chipotle peppers. Use an immersion blender, food processor, or countertop blender to puree the peppers and adobo sauce until smooth. You can either freeze them in one small freezer bag or plastic container or freeze single tablespoons of the mixture in an ice cube tray. Each tablespoon equals about one chipotle pepper, so the next time you have a recipe that calls for one, you’ll have them on hand.

    Thinly sliced Yukon Gold potatoes are baked to crispy perfection in this spicy chipotle potato tian recipe.

    • Prep Time: 15 minutes
    • Cook Time: 1 hour 30 minutes
    • Total Time: 1 hour 45 minutes
    • Yield: 4-6 servings


    • 2 lbs. Yukon Gold potatoes, thinly sliced (1/8 of an inch is good–using a mandoline slicer or food processor is best)
    • 1-2 tsp. minced or pureed chipotle in adobo sauce (1 tsp. is fairly mild, so adjust according to your own taste)
    • 3 tbsp. olive oil
    • salt to taste


    1. Preheat oven to 375 degrees.
    2. Whisk together chipotle and olive oil. Brush the bottom and sides of a small baking dish with 1/3 of the oil mixture.
    3. Place potatoes standing up in baking dish. You want them to be as tightly packed as possible. Brush tops of potatoes with remaining oil mixture and salt to taste.
    4. Bake for 75-90 minutes or until tops of potatoes are browned and crispy.


    It’s important to pack the potatoes into your baking dish as tightly as possible. To make sure you’re using the right size, before you slice the potatoes, place them in your baking dish to get an idea of how they’ll fit. (I’m using an 11 1/2-inch oval baking dish.)
